Social Prescribing

Social prescribing is a way for local agencies to refer people to a link worker. Link workers give people time, focusing on ‘what matters to me’ and taking a holistic approach to people’s health and wellbeing. They connect people to community groups and statutory services for practical and emotional support.

Are you aged 11-17 and feeling anxious/worried, left out, or struggling with the pressures of school/friends or social activities?

A Social Prescribing link worker can listen and help you unravel your thoughts, introducing you to new services and support from like-minded people.

Talk to your GP about how a social prescribing link worker could help you.
