Medication Reviews
We are keen to ensure that patients with ongoing medical problems are monitored regularly. If the date for your next review with a doctor or nurse has passed (please find this date clearly marked on the right hand side of your prescription sheet) you will be asked to make a medication review appointment. This date will usually fall around your birthday. Please help avoid delays in getting medication by completing an eConsult medication review questionnaire in the 4 weeks either side of your birthday. This questionnaire will be passed onto the practice pharmacist who will contact you should there be any issues with the information and data that you provide.
