From Sep 2021, NHSDigital is updating the system that they currently use to share some of your data held on your GP record. The General Practice Extraction Service (GPES) is being replaced with the broader and more general-purpose General Practice Data for Planning and Research service (GPDPR).

It is your responsibility to be aware of any implications of opting out, for example, some staff providing care for you may not be able to access your important medical info to ensure that the care they give is safe and effective for you.
If you wish to opt out of , there are different ways that you can do this:
- via the NHS website – https://www.nhs.uk/your-nhs-data-matters/manage-your-choice
- via the NHS App (go to the ‘your health’ section, then ‘choose if data from your health and care records is shared for planning and research’)
- download and complete the paper opt out form and post it to the practice – https://nhs-prod.global.ssl.fastly.net/binaries/content/assets/website-assets/data-and-information/data-collections/general-practice-data-for-planning-and-research/type-1-opt-out-form.docx
- call 0300 3035678 to request a paper copy of the above form to be sent to you, then complete this and post it to the practice.
